Seems the `HttpContext` attributes Map is only needed by AM.
So it's quite harmless to clear its content (that contains self-referencing
objects) before usage.

This change is a follow-up on CHF-34 change (`Promise<Response, NeverThrowsException>`).
There is no more any exception to use in the `ExceptionFilter`.
In other words, `ExceptionFilter` is useless in this new context.

User of `ExceptionFilter` are encouraged to use a `SwitchFilter` instead
(leveraging the response status in the condition expression):

Documentation has been cleaned out too.

Since we introduce the class Status in forgerock-http-framework, some code's
modifications have to be done :
- To get the code of the status we now have to do response.getStatus().getCode()
into of response.getStatus(). (in the Groovy script or EL expression that means
we'll have to write response.status.code.
- Furthermore, the attribute reason in Response does not exist any more, so it
is not possible anymore to assign a custom reason phrase, as it is set to
standardized reason phrase through the constants defined in the class Status.
- Any status code has to be in the range [100,1000[

Here we were logging all non-2xx responses, that is un-needed since they
are valid HTTP responses.
So, by filtering out the exceptions that has no parent cause, we only select
exceptions from the "transport layer".
